چکیده

Formalizing eligibility criteria in a computer-interpretable language would facilitate eligibility determination for study subjects and the identification of studies on similar patient populations. Because such formalization is extremely labor intensive, we transform the problem from one of fully capturing the semantics of criteria directly in a formal expression language to one of annotating free-text criteria in a format called ERGO annotation. The annotation can be done manually, or it can be partially automated using natural-language processing techniques. We evaluated our approach in three ways. First, we assessed the extent to which ERGO annotations capture the semantics of 1000 eligibility criteria randomly drawn from ClinicalTrials.gov. Second, we demonstrated the practicality of the annotation process in a feasibility study. Finally, we demonstrate the computability of ERGO annotation by using it to (1) structure a library of eligibility criteria, (2) search for studies enrolling specified study populations, and (3) screen patients for potential eligibility for a study. We therefore demonstrate a new and practical method for incrementally capturing the semantics of free-text eligibility criteria into computable form.

A pragmatic method for electronic medical record-based observational studies: developing an electronic medical records retrieval system for clinical research

OBJECTIVE The use of electronic medical record (EMR) data is necessary to improve clinical research efficiency. However, it is not easy to identify patients who meet research eligibility criteria and collect the necessary information from EMRs because the data collection process must integrate various techniques, including the development of a data warehouse and translation of eligibility crite...

Structuring Clinical Trial Eligibility Criteria with the Common Data Model

This paper presents a method for classifying and structuring free-text clinical trial eligibility criteria using the OMOP Common Data Model (CDM). Our method was applied to eligibility criteria text available from the largest clinical trial repository ClinicalTrials.gov. Structurally complex criteria were simplified and rewritten as simpler sentences connected by logical operators: AND or OR. S...

Towards Symbiosis in Knowledge Representation and Natural Language Processing for Structuring Clinical Practice Guidelines

The successful adoption by clinicians of evidence-based clinical practice guidelines (CPGs) contained in clinical information systems requires efficient translation of free-text guidelines into computable formats. Natural language processing (NLP) has the potential to improve the efficiency of such translation. However, it is laborious to develop NLP to structure free-text CPGs using existing f...

Towards Automatic Patient Eligibility Assessment: From Free-Text Criteria to Queries

The presented work contributes to bridging the representation of clinical trials and patient data. Our ultimate goal is to support the trial recruitment, by automating the process of formalizing eligibility criteria of clinical trials, starting from free text of criteria and leading to a computable representation. This paper discusses the final step in the pipeline i.e. generating queries from ...
